At
Elizabeth Jekot MD Breast Imaging Center, your breast biopsy
will be performed under imaging guidance using a vacuum-assisted
needle biopsy device. This type of needle allows more efficient,
quality sampling with a single insertion.
The radiologist will determine the appropriate imaging
guidance technique to use on a case-by-case basis. There
are three (3) imaging modalities we can use: sonographic
(ultrasound) guidance, stereotactic guidance, or MRI guidance.
Ultrasound guidance – Used for
sonographically visible lesions
The radiologist will use ultrasound imaging to locate the
suspicious area to be biopsied. After locating the area,
the radiologist will continue to view the area with “real-time”
sonographic guidance while numbing your breast, taking tissue
samples, and placing a small metallic clip into the biopsy
cavity. This “real-time” imaging provides for continuous
visualization throughout the procedure.
During the procedure, you will be lying on your back and
local anesthetic will be used to numb the area of the breast
to be biopsied. You will not be sedated or put to sleep for
this biopsy. Following the procedure, a two view mammogram
will be taken to visualize clip placement. After the mammogram,
the biopsy site will be dressed and your chest will be bound
with an ace wrap to minimize movement. We recommend you refrain
from heavy lifting and vigorous activity for 24-48 hours
following your biopsy.

Stereotactic guidance – Used for microcalcifications or
when the lesion is best visualized mammographically
Stereotactic biopsies are performed on a table specifically
designed for the procedure. You will lie on your stomach
on the table and your breast will hang through an opening
in the table. Beneath the table is a mammography unit, with
compression, which allows the technologist to position your
breast so the suspicious area to be biopsied is placed within
a targetable window. Once the technologist has successfully
positioned your breast, the compression will not be released
until the procedure is complete. The total time in compression
can range from 5-15 minutes. The technologist will make every
effort to help you find a comfortable position for the duration
of the procedure.
Once you are in the proper position and images confirm the
abnormality is targetable, the radiologist will clean and
numb your breast. Once you are numb, the radiologist will
place the biopsy device in your breast and begin taking samples.
Verification images will be taken periodically to assure
the biopsy needle aligns appropriately with the lesion. Before
removing the biopsy device, a small metallic clip will be
placed into the biopsy cavity.
You will not be sedated or put to sleep for this biopsy.
Following the procedure, a two view mammogram will be taken
to visualize clip placement. After the mammogram, the biopsy
site will be dressed and your chest will be bound with an
ace wrap to minimize movement. We recommend you refrain from
heavy lifting and vigorous activity for 24-48 hours following
your biopsy.

MRI guidance – Used for when the lesion best visualized
with MRI imaging
As the name implies, the MRI guided breast biopsy will be
performed in the MRI suite. A diagnostic breast MRI must
have been performed prior to scheduling for an MRI guided
breast biopsy. Because many factors contribute to your MRI
findings and interpretation, we may request a repeat MRI
prior to performing an MRI biopsy if the original MRI was
performed at another facility.
Before your MRI guided breast biopsy, an IV will be started
with a slow saline drip to afford the technologist easy access
when it is time for the contrast injection. You will lie
on your stomach on the scanning bed with your breasts hanging
through openings in the table. The breast to be biopsied
will be placed in mild compression. The technologist will
scan your breasts. After the first two scans, the technologist
will pull you out of the magnet. It is EXTREMELY important
to remain still the entire time as any movement will alter
your positioning and the procedure may have to be cancelled.
The radiologist will locate and mark the lesion and the
technologist will set the machine to the appropriate coordinates.
The radiologist will then clean and numb your breast. Once
you are numb, a plastic marking device will be placed in
the breast and the technologist will perform another scan.
Following the scan, the radiologist will verify the suspicious
area is appropriately targeted and then start taking tissue
samples. After sampling is complete, a small metallic clip
will be placed in the biopsy cavity and one additional scan
will be performed.
You will not be sedated or put to sleep for this biopsy.
If you would like something to relax you, however, please
request anti-anxiety medication for this procedure when scheduling
with our office. If approved, Dr Jekot will call a prescription
in to your pharmacy. YOU WILL NEED SOMEONE TO DRIVE YOU TO
AND FROM YOUR APPOINTMENT IF A PRESCRIPTION IS REQUESTED
AS THE MEDICATION COULD ALTER YOUR ABILITY TO DRIVE - NO
EXCEPTIONS.
